NYPL lists date as "1910?-1919?" see below. From the collection of the Flonzaley Quartet. Copy borrowed from Mills Music Library Wisconsin. If a manuscript copy, not a perfect one (see page 28/"N" in the first movement, where the copyist missed the fact that a change to treble clef was needful in the viola part!- not the only such example.)
